I leave on 1.6.21 and I contacted the department that issues the health declaration form and was kindly informed that they no longer issue those.
Now, they’re issuing QR codes which are supposed to be faster.
I contacted them to figure out what kind of timeline I needed to create from the time I get tested to the time I board the plane.
They replied with a few emails - 1 with links to articles about what to do next, another about the QR code application and a third with a “hypothetical departure” example.
Today, I created an account in advance so I can simply log in when the time comes to apply for the code.
